Social Worker

• Provide social and emotional support to residents and understand their individual needs.

• Conduct regular interactions with residents and monitor their social and emotional well-being.

• Assist in developing individual care and rehabilitation plans in coordination with the care team.

• Coordinate with family members and maintain regular communication regarding residents.

• Support residents in maintaining family and community relationships.

• Conduct counselling and supportive sessions as required.

• Maintain resident case files, assessments, progress notes, and other documentation.

• Coordinate with doctors, therapists, caregivers, and other professionals for comprehensive resident care.

• Support residents in accessing government schemes, benefits, and other available services.

• Organise recreational, social, and community-based activities for residents.

• Identify social, behavioural, or family-related concerns and report them to the Manager.

• Assist the Manager in resolving resident and family concerns.

• Participate in awareness programmes, outreach activities, and community engagement initiatives.

• Maintain confidentiality and ensure residents are treated with dignity, respect, and compassion.
