 The

Practice

Coach

provides practice transformation support to Health Center Organizations (HCOs) to help them establish and reach clinical, operational, and/or quality improvement goals; adopt best practices; implement innovations; and improve the experience and delivery of care for both patients and clinicians.

Please note: This position is funded through a contract. Continued employment is contingent upon contract renewal or extension, and organizational needs.

• Essential Duties:

•  Provide direct coaching support to clinical teams, both in-person and remotely; facilitate peer learning sessions, webinars, office hours, and technical briefings;  
• Assist care teams in obtaining and using data to create clinical/operational action plans, drive decision making, and support adoption of evidence-based care pathways. 
• Use strategies of adaptive change support, motivational interviewing, trauma responsiveness, and quality improvement to identify needs, clarify roles, and provide guidance for improving clinical and organizational processes. 
• Support health center team adoption of technology solutions to drive practice transformation along the building blocks of primary care 
• Manage multiple concurrent clinical improvement projects, actively participate in OCHIN cross-departmental project teams, and build/maintain relationships with internal and external stakeholders to improve OCHIN’s systems and processes 
• Identify, document and share best practice insights and actionable recommendations to internal and external stakeholders
