Holon Health is seeking a compassionate and highly organized Case Manager to join our collaborative care team. The Case Manager will play a crucial role in proactively monitoring client progress, ensuring adherence to treatment plans, and providing support to facilitate positive treatment outcomes. This role also coordinates closely with Justice Liaisons from health plans and with facility managers at correctional sites to schedule both in person and telehealth appointments. This position offers a unique opportunity to make a significant impact on the lives of individuals in recovery and contribute to a patient centered, integrated care model.
